IMPORTANT INFORMATION FOR APPLICANTS

Thank you for your interest in the Colorado Traumatic Brain Injury Trust Fund. The following information is provided to help you decide whether this program is right for you. Please read it carefully before completing the application.
PROGRAM ELIGIBILITY
Traumatic Brain Injury (TBI) is defined as an injury to the brain caused by external force. Causes of TBI include, but are not limited to, falls, motor vehicle accidents, rapid acceleration/deceleration, sporting events and assaults. Brain injuries caused by anoxia, stroke, aneurysm, congenital abnormality, disease, and surgical intervention are not included in this definition and do not qualify for assistance from the Colorado TBI Trust Fund.
To be eligible for the Colorado TBI Trust Fund program, you must meet all of the following criteria:
- Be a legal resident of the United States and Colorado.
- Have a medically documented Traumatic Brain Injury, resulting in ongoing impairment of cognitive or physical function.
- Complete and return the application and required documentation.
The Colorado TBI Trust Fund is not an entitlement program. While qualified applicants are eligible to receive Trust Fund assistance, they are not legally entitled to the services offered by the program.
LIFETIME LIMIT TO SERVICE
Eligible applicants will be served on a first come, first serve basis and may be placed on a waiting list until funds are madeavailable. In order to reduce the waiting time and serve as many individuals as possible, the Colorado TBI Trust Fund has established the following service limitations:
- Effective March 1, 2007, a participant may only receive care coordination and services for a total of one year during his/her lifetime.
- If you received services on or before March 1, 2007, you may apply for one additional year on the program. You must complete your current year of services before reapplying, and will be placed at the end of the waiting list once approved.
You are encouraged to carefully consider your current situation and available resources before choosing a time to use this once-in-a-lifetime benefit. If the lifetime limit to service is eliminated in the future, clients will be notified of the change by mail.
CARE COORDINATION & SERVICES
Care coordination is the focus of the Colorado TBI Trust Fund Program. Your care coordinator will work with you to assess your situation and develop a care plan to be followed during the year. The care coordinator will identify resources to meet your needs, and will provide the support and assistance necessary to access these resources.
Your care plan may also include purchase of up to $2,000 in items or additional services. These funds are restricted as follows:
- The need for purchased items/services must be related to your Traumatic Brain Injury.
- You must have no other means of obtaining these items/services.
- All items/services must be included in your Colorado TBI Trust Fund care plan before they are purchased or received. Under no circumstance will the program pay for past bills or purchases.
- Purchases must be made within the program year, which begins on the day the care plan is signed by both you and your care coordinator.
- Providers are paid for items/services directly. The program will not pay or reimburse individual clients or their families.
- Purchases are subject to the availability of qualified providers, who are willing to see the client and accept the established payment rate for items/services. Providers must obtain written authorization from the care coordination agency before delivering items/services to the client, in order to be paid by the Colorado TBI Trust Fund.
- The Colorado TBI Trust Fund cannot pay for rent, utilities, medications, hospitalization, institutionalization, food, or cash.

This program is designed to provide funding for items and services made necessary by a client’s traumatic brain injury, but not covered by other funding sources. Your care coordinator will work with you to make decisions about using these funds to most benefit you. To maintain flexibility in the program, there is no comprehensive list of items and services that are paid for by the Colorado TBI Trust Fund. However, the following are examples of the kinds of things that may be provided:
- Activities of daily living
- Acupuncture
- Adaptive devices
- Adult recreation/camp programs for individuals with disabilities
- Alternative communication devices
- Assistive technology and devices
- Biofeedback
- Chiropractic treatment
- Cognitive therapy
- Community residential services
- Companion services
- Co-payments for TBI related services covered by insurance
- Driver evaluation and training
- Durable medical equipment
- Educational materials
- Employment/vocational rehabilitation services
- Exercise equipment or programs
- Hearing aids, filters or treatment for hearing disorders
- Home and environmental modifications
- Home care
- Independent living skills training and development
- Massage
- Neuromedical treatment
- Nutrition and diet consultation
- Occupational therapy
- Organizational supports
- Patient, family and support system advocacy
- Personal care
- Physical therapy
- Psychological and mental health services
- Respite care
- Socialization skills training
- Speech and language therapy
- Structured day program services
- Substance abuse treatment
- Supported living programs
- Therapeutic recreation
- Transitional living services
- Transportation to and from services included in the care coordination plan
- Tutoring
- Vehicle modifications
If your desired item or service is not on this list and you are not sure whether it will be covered by the program, you may discuss your needs with an intake coordinator prior to applying for the program.
